Sports Physicals for grades 7-12 (2021-22) will be given on the following dates:
Monday, June 14th from 3:00 PM to 6:00 PM
Tuesday, June 15th starting at 3:00 PM
Physicals will be given at:
Dr. Cienciva’s office, located at 1095 Million Dollar Hwy.
Please note that if you cannot make the June 14th or June 15th date, you will be responsible for scheduling an appointment with your family physician, Q-Care, or another medical provider.
Sports physical packets can be picked up at the main office at Elk County Catholic High School or printed from the ECCSS website under Athletic Forms. Link to the physical forms: https://www.eccss.org/athletics-forms/
All packets MUST BE COMPLETED BY PARENTS PRIOR TO ARRIVAL on the day of the sports physicals. Please make sure all paperwork is complete and signed both by the parent & student athlete BEFORE arrival. Any students arriving without properly completed paperwork will not be given a physical. It is recommended that parents attend the physical with their student-athlete in the event that the provider has any medical information to discuss.

Parents please note that in order to meet the PIAA regulations for the 2021-2022 school year, physicals must be given AFTER June 1, 2021. Also, please note that the physical forms have been updated for the 2021-2022 academic year. Page #1 of the CIPPE forms state.
